Why does neck skin start to look crepey – and can it look smoother again?

Crepey skin on the neck showing fine lines and changes in skin texture

You may have a skincare routine that works well for your face, then start noticing something different on your neck.

The skin can look thinner and less smooth. Fine creases appear. What used to disappear with movement can become more noticeable even when your neck is relaxed.

This is often described as crepey skin.

But crepey skin doesn’t look the same at every stage. The changes tend to build gradually, and what may help depends on how much the skin has changed.

1. The first changes are usually in the skin’s surface

What you may notice: fine creases and a slightly crinkled texture, particularly when you turn or bend your neck. The skin may also feel drier than it used to.

What’s changing: the skin becomes less effective at holding moisture, while gradual changes in collagen and elastin affect its firmness and elasticity.

What may help: consistent moisturising, daily sun protection and skincare suited to your skin are still important at this stage. They help support the skin barrier and reduce further UV damage.

The focus here is on maintaining skin quality while the changes are still subtle, rather than trying to correct more established changes later.

2. The fine creases become more noticeable

What you may notice: the crinkled texture is now visible even when your neck is relaxed. Horizontal lines may become more obvious and the skin no longer looks as smooth as it once did.

What’s changing: dryness is no longer the only factor. Changes in collagen and elastin mean the skin doesn’t spring back as readily, so repeated creases can become more noticeable.

What may help: home care still matters, but skincare alone may not create the degree of change you’re looking for.

Treatments that encourage skin renewal and collagen remodelling may be considered, including microneedling and fractional radiofrequency.

At LSV, Venus Viva is one of the treatments used to address skin texture and overall skin quality.

The aim at this stage is simple: to make the crepey texture less noticeable and help the neck look smoother again.

3. The skin starts to lose firmness as well

What you may notice: the issue is no longer just fine lines. The skin can start to look less firm, and the neck may lose some of the smooth, supported appearance it had before.

What’s changing: changes in the skin are now affecting both texture and firmness. Improving the surface alone may not be enough to create the result you want.

What may help: treatments that address the appearance of both skin texture and firmness may be considered.

Radiofrequency-based treatments are one option. At LSV, Venus Legacy is used on areas including the face, neck and jawline.

At this stage, the goal shifts from simply softening fine lines to helping the skin look firmer, smoother and fresher overall.

4. The changes start to affect the neck and jawline

What you may notice: the jawline becomes less defined, the angle between the chin and neck begins to soften, or vertical neck bands become more visible.

What’s changing: at this point, the overall appearance is being influenced by more than the skin alone. Deeper tissues also contribute to the shape of the neck and jawline.

What may help: non-surgical treatments may still improve certain concerns, including skin texture and the appearance of firmness.

But there is a limit to how much they can change the shape or contour of the neck. They cannot produce the same degree of structural change as surgery.

The goal at this stage is usually broader: improving the definition of the neck and jawline, not just the texture of the skin.

This is why two people who both say, “I just want my neck to look smoother”, may need very different advice.

With earlier changes, the focus may be on maintaining skin quality and improving texture.

As the changes become more established, the more important question is not which treatment sounds best, but how much improvement that treatment can realistically achieve.

That’s what we look at during a consultation.

We assess the skin, how established the changes are and what result you’re hoping for. From there, we can explain which options may be worth considering — and which may not give you enough change to be worthwhile.
